Returning membership enrollment form

Thank you for your interest in re-enrolling as an Adobe® Open Options CLP Commercial member. The next step is to complete and submit the enrollment form.

This form is for commercial organizations wishing to renew their CLP membership, which is either currently in effect or has expired within the past six months.

The requirements and conditions for re-enrollment are governed by certain program terms. Contact your Adobe account representative or your Adobe License Center (ALC) to determine your transition status and appropriate CLP transition level.

If you are a new CLP member, or you want to join CLP as a Self-Enrolled Affiliate, select "New CLP Commercial membership" or "Self-Enrolled Affiliate" from this page.

If your organization is not a commercial entity, do not submit this form. Instead, return to the CLP 4.5 page and select the appropriate membership category for your organization.

The Enrollee is the individual who enrolls in the CLP program on behalf of the Program Applicant. The Enrollee will be contacted only if Adobe has any questions on this enrollment form. The Enrollee may differ from the "Contact" under the Program Applicant section of this form. The "Contact" under the Program Applicant section will be listed as the Primary Contact on the CLP agreement and will receive all upgrade notices and program information from Adobe.
